Purpose: To evaluate visual function after bilateral phacoemulsification and implantation of apodized diffractive acrylic intraocular lenses (IOLs) with a +3D or +4D addition power.

Methods: This is a prospective interventional study. Phacoemulsification was done and Acrysof IQ Restor IOLs were implanted bilaterally. Patients were divided into 3 groups: patients in group 1 were implanted bilaterally with +3D addition power SN6AD1 IOLs, group 2 implanted bilaterally with +4D SN6AD3 IOLs, and group 3 implanted with +3D SN6AD1 IOL in one eye and +4D SN6AD3 IOL in the other eye. Distant, near, intermediate visual acuity, contrast sensitivity and subjective evaluation of patients on visual disturbances, spectacle dependence and satisfaction were assessed.

Results: Of 43 patients, 12 patients were in group 1, 11 in group 2 and 20 in group 3. The bilateral intermediate visual acuity at 50cm and 60cm distance in group 1 was statistically significantly better than in group 2 and 3, while no statistically significant difference in group 2 and 3. Distant, near and intermediate visual acuities at 70cm distance were not different statistically significantly. Visual disturbances were rare in 3 groups and there was no difference significantly. Excellent satisfaction and spectacle independence were reported.

Conclusions: Bilateral implantation of apodized diffractive acrylic IOLs with a +3D
addition power provided better intermediate visual acuity at the distance 50cm and 60cm than bilateral +4D IOLs or combination of one eye implanted +3D IOLs and the other eye +4D IOLs. Patients implanted both types of these apodized diffractive IOLs reported excellent visual quality, spectacle independence and satisfaction.
